Long names in the problems index can blow up the table layout, causing ugliness and potentially horizontal scrolling. This change adds whitespace: nowrap and sets a few td widths to keep the layout from jumping around.

In the past users could only follow apps if an admin did it for them. But now users can follow and unfollow any apps. Following an app is no longer like a permission to view that app.

On error list page, comments with especially long words (typically : urls) break layout. Fixed it by using the same forced width and wrapping settings than on error message.

This means that in a production environment the images will use a digested filename. We can therefore safely set far-future expires headers for them.
